Job summary

Nutrition that Works, LLC seeks a consultant dietitian to support our long-term care community in Lansdale, PA, 16-24 hours per week under a maternity coverage arrangement. The role focuses on clinical nutrition assessments and care planning within the Nutrition Care Process.

You will document in the EMR, collaborate with an interdisciplinary team, complete MDS - Section K, calculate TF, and provide wound and weight-change recommendations while maintaining a patient-centered approach and strong

Qualifications

• Credentialed by the CDR.
• Dietetic licensure.

Responsibilities

• Completing clinical nutrition assessments.
• Documenting using the Nutrition Care Process in the EMR system.
• Collaborating with the interdisciplinary care plan team.
• Completing the MDS - Section K.
• Calculating TF.
• Making recommendations for wounds and significant wt changes.
