Job description

Under the supervision of the CEO, the Outreach Worker works with Baltimore and the surrounding area to build relationships with underserved neighborhoods, provide advocacy, and assist with connecting individuals to Citywide Behavioral Health services and housing. The Outreach Worker must have previous experience working with individuals experiencing homelessness, addiction, and mental health issues. The Outreach Worker identifies individuals through direct street outreach activities, organization, and networking. The Outreach Worker will work a conventional work week that includes evening hours and may require weekend work.

In identifying highly vulnerable neighbors experiencing mental health, addiction, and homelessness, the Outreach Worker will gather the necessary information and bring it back to CBH to review to see if the individual is appropriate for our facility. If so, arrange to get the client to the CBH center. The Outreach Worker must be detail-oriented, have strong follow-up skills, and must be able to communicate and connect with the community.

ESSENTIAL DUTIES
  1. Visit communities and canvass neighborhoods.
  2. Work collaboratively with the CBH team to conduct outreach in the community, focusing on those who are most vulnerable.
  3. Visit with neighbors experiencing homelessness, addiction, and mental health issues and discuss how CBH can assist them with their current situation.
  4. Continue to engage with the community, organizations, and connections to maintain that the residential housing beds are full.
  5. Assist clients with all necessary paperwork and answer all questions that will assist the client in making the decision to enter the CBH program.
  6. Maintain computerized client records, daily activity logs, mileage logs, and other reports as directed.
  7. May drive a van/car; complete daily vehicle inspection report on van/car; may clean van/car; and may maintain all safety conditions in the van/car.
  8. Develop a relationship with the community through frequent outreach visits to foster trust building and support participation in CBH programs.
  9. Encourage and support client participation in activities and events that increase their ability to remain linked to care and/or behavioral health care.
  10. Provide support to clients when they encounter barriers.
  11. Assist with the planning and implementation of services that meet the clients’ needs to become successful at CBH programs.
  12. Attend monthly community outreach meetings.
  13. Other duties as assigned.
QUALIFICATIONS
  1. Experience in social services or a related field accepted with a minimum of two (2) years of case management experience or social services experience will be considered.
  2. Flexibility (Position will require evenings occasionally, up to twice a week, no later than 8pm).
  3. Thorough knowledge of reference and research methods and techniques used in collecting, compiling, and organizing data.
  4. Demonstrated ability to explain an agency's purpose, programs, and operations.
  5. Knowledge of available community resources.
  6. Effective communication skills.
  7. High School diploma or equivalent.
  8. Must be able to read and write English and follow oral and written instructions.
  9. Must pass drug and background checks.
