Qualifications

• Board certification/eligibility in Internal Medicine, Family Medicine, or Med-Peds.
• Indiana medical licensure eligibility.
• Minimum eight years of post-residency clinical experience preferred.
• Commitment to patient-centered, relationship-based care.
• Strong communication and interpersonal skills.

Responsibilities

• Deliver comprehensive primary care through a concierge model.
• Develop individualized treatment, prevention, and wellness plans.
• Manage acute and chronic medical conditions.
• Coordinate care across specialties and healthcare settings.
• Provide accessible service via phone, text, email, telehealth, and office visits.
• Build long-term trusted relationships with patients and their families.
• Participate in after-hours patient communication and care needs.
