Job summary

Ohana Pacific Health is seeking a Social Services Assistant to partner with the interdisciplinary team in Hilo, providing psychosocial support and planning care for Kupuna. You will assess emotional, environmental, and spiritual needs and help develop a plan of care.

The role involves counseling during crises, conducting rounds, completing initial and timed assessments, and coordinating guardianship processes as needed. 1+ year in social services or long-term care is preferred.

Qualifications

• Initial assessments, state/federal timed assessments, and care plans.
• Experience providing social services or in long-term care preferred.
• Membership in NASW and/or Academy of Certified Social Work preferred.

Responsibilities

• Assist with counseling and crisis intervention to support coping and problem solving.
• Make daily rounds and review social service logs and care records.
• Complete social services processes including assessments and care plans.
• Identify guests needing POA, surrogates or guardianship and coordinate the process.
