T.O.N.E. Home Health Services in Flint, MI, is seeking a compassionate Speech-Language Pathologist (SLP) to provide home health services across the Greater Flint area. This role involves performing evaluations, developing treatment plans, and collaborating with a supportive team.

Qualifications

• Minimum 1 year of healthcare experience preferred (home health experience strongly preferred).
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Perform comprehensive speech, language, cognitive, and swallowing evaluations.
• Develop individualized treatment plans based on patient needs and physician orders.
• Provide skilled therapeutic interventions to improve communication, swallowing, and functional independence.
• Educate patients, caregivers, and families on exercises, strategies, and home programs.
• Collaborate closely with interdisciplinary team members.
• Complete accurate, timely documentation in compliance with relevant standards.
