Job summary

ASHFORD CONNEX, LLC is seeking a full-time Registered Nurse (RN) to join our home health team in Casper, WY. You will provide high-quality patient care to seniors in their homes, delivering care in a compassionate and professional manner.

The RN will assess needs, develop care plans, administer medications, educate patients and families, and collaborate with an interdisciplinary team to ensure coordinated services. Valid Wyoming RN license and a valid driver's license are required.

Qualifications

• Current RN license in the state of Wyoming.
• Minimum of 0 years of experience in home care or a related setting.
• Ability to work flexible hours, including evenings, weekends, and holidays.
• Valid driver's license and reliable transportation (mileage reimbursed).

Responsibilities

• Provide skilled nursing care to seniors in their homes, assessing their physical, emotional, and social needs.
• Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary team members.
• Administer medications, treatments, and therapies as prescribed by physicians, and monitor patient responses.
• Educate patients and families on disease management, self-care, and wellness strategies.
• Collaborate with other healthcare professionals to ensure comprehensive care and coordination of services.
• Maintain accurate and detailed records of patient interactions, assessments, and care plans.
