Responsibilities

• Provide direct patient care, including assessing, planning, implementing, and evaluating care plans for residents.
• Administer medications, treatments, and therapies as prescribed by physicians and according to established protocols.
• Monitor resident progress, identify potential complications, and take corrective action as needed.
• Develop and implement individualized care plans, including setting goals and documenting progress.
• Collaborate with interdisciplinary team members to ensure seamless care coordination and communication.
• Communicate effectively with residents, families, and healthcare team members to ensure understanding of care plans and resident needs.
• Monitor and update electronic medical records to ensure accuracy and completeness.
