Responsibilities

• Performs a comprehensive assessment on all assigned patients with documentation.
• Communicates changes in patient's condition promptly to the care team.
• Derives realistic outcomes from nursing diagnoses and documents care plan goals.
• Identifies patient needs and collaborates with family and health care team for planning.
• Assesses discharge planning needs and makes appropriate referrals as necessary.
• Evaluates patient responses to interventions and adjusts the plan of care.
