Responsibilities

• Provide high-quality patient care to residents, including administering medications, wound care, and treatments as prescribed by physicians
• Develop and implement individualized care plans for each patient, taking into account their unique needs and preferences
• Conduct initial and ongoing assessments of patients' physical and emotional needs, identifying opportunities for improvement and implementing strategies to address them
• Collaborate with other members of the care team to ensure seamless communication and coordination of patient care
• Participate in quality improvement initiatives to enhance patient satisfaction, reduce readmissions, and improve clinical outcomes
