Job summary

Palms-Home-Care in Florida is seeking a dedicated Physical Therapist Assistant (PTA) to support patients under the supervision of a licensed Physical Therapist, delivering high-quality, patient-centered care in home health settings.

You will implement treatment plans, administer exercises and modalities, monitor progress, educate families, document records, and collaborate with RNs, PTs, OTs, and Speech Therapists to ensure coordinated care.

Qualifications

• Associate degree in Physical Therapist Assisting or equivalent.
• 1 year of home health experience required.
• Current state licensure as a PTA.
• Excellent communication and collaboration with patients and healthcare teams.

Responsibilities

• Implement treatment plans designed by the supervising Physical Therapist, ensuring effective, individualized care.
• Administer therapeutic exercises and modalities for patients in geriatrics, orthopedics, pediatrics, outpatient, and acute care settings.
• Observe, assess, and document patient responses and progress, reporting changes or concerns promptly to the supervising PT.
• Educate patients and families on home exercise programs, safety precautions, and techniques to promote continued recovery.
• Maintain accurate and thorough treatment records, progress notes, and other required documentation.
• Collaborate with multidisciplinary team members including RNs, PTs, OTs, and Speech Therapists to ensure coordinated, holistic care.
• Promote and maintain a safe, clean, and supportive environment for all patients.
