You will document care electronically via tablet, coordinate with care teams, and contribute to consistent, compassionate home-based support across shifts.

Qualifications

• Active LPN/LVN license in the state of practice.
• Current CPR certification.
• Six months hands-on nursing experience preferred.

Responsibilities

• Provide home-based nursing care to adult patients, including high-acuity cases (gtube, trach, vent).
• Document patient care electronically using tablet-based systems.
