Job summary

Infinity Hospice Care LLC is seeking Home Health Aides to provide compassionate in-home care, assisting patients with activities of daily living and monitoring health conditions. You will support families, document care in our EMR, and coordinate with the Care Team to ensure continuity of care.

Ideal candidates have 1+ year of HHA/CNA experience, an active CPR certification, and a valid driver’s license with reliable transportation.

Qualifications

• High School graduate or equivalent.
• Home Health Aide Certificate or equivalent certification in good standing.
• Minimum of 1+ year experience as a home health aide or CNA.
• Strong English language both written and verbal.
• Active CPR certification required.
• Valid driver’s license and reliable transportation with insurance.

Responsibilities

• Monitor and record patient condition, provide support and personal services.
• Assist with activities of daily living and housekeeping tasks.
• Provide bathing, dressing and grooming assistance as needed.
• Teach families to lift, turn, and reposition the patient.
• Document patient information in EMR and notify branch leadership of changes.
• Participate in on-call responsibilities and maintain a safe care environment.
