Job summary

Evolve Therapy Services, LLC in Minot, ND is seeking a Physical Therapist for an in-house Skilled Nursing Facility on a part-time or PRN basis. You will plan, organize, develop, and direct physical therapy services to ensure high-quality resident care in compliance with standards.

Responsibilities include screening residents, developing treatment plans, documenting per CMS and NetHealth EMR, and collaborating with the interdisciplinary team.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop treatment plans with short and long-term goals and monitor progress.
