Job summary

Trinity Healthcare Talent Services is seeking a skilled Physical Therapist to join a SNF/ALF team. You will assess patients, design personalized treatment plans, and guide recovery to restore mobility and independence.

The role requires licensed PT with 1–2 years of experience and strong knowledge of anatomy, physiology, kinesiology, and collaboration with a multidisciplinary team. Weekend availability may be required.

Qualifications

• Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• Minimum of 1-2 years of experience working as a Physical Therapist, preferably in a SNF or ALF setting.
• Strong knowledge of anatomy, physiology, and kinesiology.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate and patient-centered approach to care.
• Flexibility to work occasional weekends and evenings as needed.

Responsibilities

• - Conduct initial evaluations and develop individualized treatment plans for patients based on their specific needs and goals.
• - Implement and monitor treatment plans, adjusting as necessary to ensure progress and optimal outcomes.
• - Use a variety of therapeutic techniques, exercises, and equipment to help patients improve their strength, flexibility, balance, and coordination.
• - Educate patients and their caregivers on proper techniques for performing exercises and activities to promote long-term wellness and prevent future injuries.
• - Collaborate with other healthcare professionals, such as occupational therapists and physicians, to provide comprehensive care for patients.
• - Maintain accurate and timely documentation of patient progress, treatment plans, and other important information.
• - Stay updated on the latest advancements and best practices in physical therapy to continuously improve patient care.
