Liberty Healthcare and Rehabilitation Services is seeking an experienced Physical Therapist (PT) to join our team in a SNF setting. You will work with a multidisciplinary group to develop and implement individualized treatment plans, focusing on progressive modalities to address a variety of diseases and impairments.

The role emphasizes in-house therapy with supportive local management, ensuring compassionate, high-quality care for residents and their families.

Qualifications

• Must be a graduate of an accredited therapy program.
• Must be licensed in the state where SNF work will be conducted.
• Previous SNF rehab experience preferred.

Responsabilités

• Collaborate with a multidisciplinary team to deliver tailored rehab plans.
• Provide in-house therapy with local management support.
