Job summary

Tarboro Health and Rehabilitation in Tarboro, NC is seeking a dedicated Physical Therapist to join our team in providing high-quality rehabilitative care to residents in a skilled nursing facility. As a PT you will develop personalized plans, collaborate with physicians and nurses, monitor progress, educate residents and families, and document outcomes to ensure optimal recovery and mobility.

Qualifications

• Current, active state license as a Physical Therapist in North Carolina.
• Graduate of an accredited Physical Therapy program (DPT or Master's degree in Physical Therapy).
• Previous SNF or geriatric care experience preferred but not required.
• Strong communication, interpersonal, and organizational skills.
• Ability to work collaboratively in a fast-paced healthcare environment.
• Compassionate, patient-centered approach to care.

Responsibilities

• Develop personalized treatment plans for residents to address physical impairments, mobility issues, and functional deficits.
• Administer appropriate physical therapy treatments to help residents regain strength, mobility, and independence.
• Monitor and document patients' progress, adjusting treatment plans as needed to achieve optimal results.
• Educate residents, families, and caregivers on therapy plans, safety precautions, and continued care.
• Collaborate with interdisciplinary teams, including physicians, nurses, occupational therapists, and speech therapists, to ensure holistic patient care.
• Ensure compliance with federal, state, and facility regulations related to rehabilitation therapy.
