Job summary

Qualifications

• 1 year of experience or equivalent training in home health care.

Responsibilities

• Provide personal care to patients, including bathing, dressing and grooming.
• Assist with daily living activities: toileting, incontinence care, mobility.
• Administer medications as prescribed by a physician.
• Develop and implement personalized care plans for each patient.
• Collaborate with healthcare teams to ensure seamless transitions and comprehensive care.
• Maintain accurate and detailed records of patient care and progress.
