Job summary

Visiting Nurse Health System in Conyers, GA, is seeking an experienced Occupational Therapist to provide direct patient care, education, and therapy management under the patient’s plan. The role emphasizes compassionate care and collaboration with the care team.

The OT will evaluate patient progress, coordinate with physicians for additional services, and maintain precise documentation. A GA license and driving essential; per-visit compensation includes a sign-on bonus and comprehensive benefits.

Qualifications

• 2 years of full-time OT experience.
• Bachelor’s or Master’s Degree in Occupational Therapy.
• GA OT License current.
• Must have a valid driver's license and proof of vehicle insurance.

Responsibilities

• Provide direct care, patient education, and management of the therapy per patient’s treatment plan.
• Consult with and educate other care team members on patient assessment and treatment protocols.
• Serve as a rehabilitation resource for the care team.
• Evaluate patient response to therapy, arrange additional services, and obtain physician orders.
• Ensure clinical documentation and Oasis time points are complete and accurate with low error rate.
• Serve as a preceptor to others when needed.
