AccentCare, Inc. is seeking a dedicated LVN/LPN to provide direct patient care under RN supervision at the Lake Jackson office. This full-time, on-site role serves pediatric to geriatric patients across Sweeny and Brazoria, with per-visit compensation reflecting an annual range.

You will administer medications, maintain medical records, and coordinate with the home health team to ensure high-quality care. Requires active LVN/LPN license and CPR certification.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of operation.
• One year of LVN/LPN work experience.
• Must possess and maintain CPR certification while employed in a clinical role.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provides direct patient care under RN supervision.
• Administers prescribed medications and treatments as permitted by law.
• Maintains accurate medical records and documentation.
• Communicates with case manager and home health team members about patient data.
• Supports nursing care plan and documents progress in a timely manner.
