Job summary

Sweetwater Care is seeking an experienced LVN to provide nursing care within scope of practice and according to physician orders and facility policy in a California-based facility.

The role emphasizes leadership in nursing management, coordination with the care team, and effective communication with families. Prior long-term care experience and proficiency with PointClickCare charting are highly valued.

Qualifications

• Must be licensed LVN in California.
• 1 year of nursing management experience preferred.
• SNF/LTC experience preferred.
• Bilingual is a plus.
• Long term care experience required.
• Point Click Care charting experience required.

Responsibilities

• Demonstrates knowledge of the Infection Control Manual.
• Participates in and assists in the Quality Improvement Program.
• Utilizes the Quality Life Manual for patient placement and monitoring.
• Notifies other departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes, and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians as necessary.
