Elara Caring is seeking a Licensed Vocational Nurse (LVN) for the Springfield, MA area to join our PRN team. The role focuses on delivering high-quality care to patients in home settings. You will follow care plans, monitor progress, and collaborate with the care team to ensure safety and positive outcomes.

Qualified candidates have an active LVN license, at least 1 year of clinical experience, and the ability to travel between patient homes. Hospice/home health experience is preferred.

Formación

• Current State License as LVN in good standing.
• Minimum 1 year of clinical care experience.
• Must be able to lift up to 50 pounds and assist with patient transfers.
• Must have valid driver’s license or reliable transportation.
• Hospice/home health experience preferred.

Responsabilidades

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Educate patients and caregivers on self-care techniques.
• Update care plans with the RN Case Manager as needs change.
• Document medication regimens and update medication profiles.
