Job summary

Leisure Vale Assisted Living in Glendale, CA is seeking a Licensed Vocational Nurse (LVN) to provide direct nursing care to residents, including medication administration and monitoring of vital signs. You will assess residents, develop individualized care plans with the interdisciplinary team, coordinate access to medical and social services, and advocate for residents' rights and housing benefits.

Maintain accurate documentation in EHRs and support families with counseling and crisis

Qualifications

• Must hold a current California LVN license.
• Demonstrates compassion and empathy for diverse residents.
• Experience in healthcare or social services preferred.

Responsibilities

• Provide direct nursing care to residents, including med administration, wound care, and monitoring of vital signs.
• Assess residents and develop individualized care plans with the interdisciplinary team.
• Coordinate access to medical and social services for residents.
• Counsel and support residents and families to promote coping and resilience.
• Advocate for residents' rights and access to housing and benefits.
• Collaborate with community partners to assist residents' transition to permanent housing.
• Maintain accurate documentation in electronic health records (EHR).
