Job summary

ATC Healthcare Services in Edison, New Jersey, is seeking a Licensed Social Worker (LSW) for a full-time day shift to provide psychosocial support, counseling, and care coordination for patients and families. Qualifications

• Bachelor's or Master's Degree in Social Work (BSW or MSW) from an accredited institution.
• Current and active Licensed Social Worker (LSW) license in the state of practice.
• Minimum of 2–4 years of social work experience required.
• Experience in healthcare, hospital, long-term care, rehabilitation, behavioral health, or community settings preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments of patients and families.
• Develop, implement, and monitor individualized care and discharge plans.
• Provide counseling, crisis intervention, and emotional support services.
• Coordinate community resources and referrals to support patient needs.
• Advocate for patients and families to ensure access to appropriate services.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals.
• Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
• Assist patients with transition planning, including discharge and post-discharge support.
• Educate patients and families regarding available resources, benefits, and support programs.
• Participate in interdisciplinary team meetings and case reviews.
