Job summary

Qualifications

• Graduate of an accredited LPN program.
• Current and unrestricted North Carolina LPN license.
• Experience in long-term care, rehabilitation, or skilled nursing preferred but not required.

Responsibilities

• Provide direct nursing care in accordance with individualized care plans and physician orders.
• Monitor residents for changes in physical, mental, emotional, and psychosocial status.
• Document assessments, observations, and care provided accurately and timely.
• Obtain and monitor vital signs, weights, glucose, intake/output, and other clinical data.
• Assist with admissions, transfers, discharges, and transitions of care.
• Participate in care planning and interdisciplinary team meetings.
• Respond appropriately to resident emergencies and changes in condition.
• Administer medications, injections, treatments, and enteral feedings as ordered.
• Complete MARs and TARs accurately.
• Monitor medication effectiveness and adverse reactions.
• Provide guidance to CNAs and other staff; delegate tasks as appropriate.
