Job summary

Qualifications
- Must have a current LPN license in the state of Tennessee.
- Previous experience in a rehabilitation or long-term care setting preferred.
- Demonstrates strong clinical and communication skills.
- Compassionate and resident-focused approach to care.
Responsibilities
- Administer medications and treatments as prescribed by physicians.
- Monitor residents’ conditions and report changes to the nursing team.
- Provide direct nursing care and support to residents.
- Maintain accurate and timely documentation in medical records.
- Collaborate with RNs, physicians, and interdisciplinary team members.
- Assist with admissions, discharges, and resident assessments.
- Ensure compliance with facility policies and healthcare regulations.
