Job summary

You will coordinate care with other disciplines, document resident information, and participate in care conferences, ensuring compliance with state and federal regulations. Current Wisconsin LPN license required; strong communication skills preferred.

Qualifications

• Must have current and active Wisconsin LPN license.

Responsibilities

• Prepare and administer medication as ordered by the physician.
• Review medication orders for completeness and accuracy.
• Transcribe physician orders.
• Document resident information and notify physician/family as needed.
• Assist in Admission/Discharge process.
• Complete accident/incident reports and follow-up investigations.
• Participate in care planning and care conferences.
• Maintain rapport with all disciplines to meet resident needs.
• Deal tactfully with personnel, residents, families, visitors, and others.
• Participate in continuing education programs.
