Job summary

AccessHealth invites a dedicated Licensed Practical Nurse (LPN) to join our long-term and post-acute care team. You will deliver person-centered care, administer medications, monitor residents, and document in our EHR system, upholding high standards of safety and compassion.

Collaborating with an interdisciplinary team, you will guide CNAs, support infection control and quality initiatives, and educate residents and families to foster understanding and comfort, within our faith-based culture

Qualifications

• Current and valid LPN license.
• Strong interpersonal and communication skills.
• Ability to work collaboratively within a team.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Deliver person-centered care, including administering medications and treatments.
• Monitor resident conditions and document accurately in Electronic Health Records (EHR).
• Collaborate with an interdisciplinary team to develop and support individualized care plans.
• Supervise Certified Nursing Assistants (CNAs) and promote infection control and safety protocols.
• Provide education to residents and their families to enhance understanding and comfort.
• Contribute to quality improvement initiatives and uphold a compassionate, faith-based culture.
• Engage in ongoing training and professional growth opportunities.
