Job summary

Pacific Care Center in Pacific, Missouri is seeking a compassionate Licensed Practical Nurse (LPN) to join our care team and support residents with daily nursing needs.

You will administer medications, monitor health status, document observations, and collaborate with a multidisciplinary team to plan, implement, and evaluate patient care. You’ll also assist with admissions, transfers, and discharge planning while maintaining a safe, respectful environment for all residents.

Qualifications

• LPN licensure (MO) required or preferred.
• Ability to work effectively as part of a multidisciplinary team.
• Strong communication and interpersonal skills.
• Ability to prioritize tasks and manage time efficiently.
• Commitment to high-quality patient care and adherence to standards/policies.

Responsibilities

• Provide nursing care to patients as assigned.
• Administer and record medications and treatments as assigned.
• Reinforce supports and provide healthcare instructions to patients and their families.
• Participate with other team members in planning, implementing, and evaluating nursing care and other patient activities.
• Assist in the admission, transfer, and discharge of patients.
• Observe and report patients' vital signs, symptoms, and general condition to the appropriate individual(s).
• Record patient charts observing patients’ responses to nursing care rendered.
• Maintain environmental conditions conducive to the comfort and safety of patients.
• Assist with unit management and/or assume charge of unit as assigned.
