Job summary

Transitions Healthcare Gettysburg is seeking compassionate Licensed Practical Nurses (LPNs) to join our award-winning team. We provide comprehensive medical, dental, and vision benefits, PTO, on-demand pay, and opportunities for professional growth.

As an LPN, you will administer medications, monitor residents, document care, and collaborate with physicians and the inter-disciplinary team to promote health and well-being in a resident-centered environment.

Qualifications

• Graduate of an accredited Licensed Practical Nursing (LPN) program.
• Current LPN license in good standing with the Commonwealth of Pennsylvania.
• CPR certification preferred.
• Strong clinical, communication, organizational, and teamwork skills.
• Commitment to providing compassionate, high-quality resident care.

Responsibilities

• Administer medications and treatments as prescribed by physicians and monitor residents for therapeutic effectiveness and adverse reactions.
• Assess, monitor, and document resident conditions, reporting changes promptly to Registered Nurses and physicians.
• Obtain and accurately record vital signs and other clinical observations.
• Maintain complete, accurate, and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Assist with admissions, transfers, and discharges as assigned.
• Communicate effectively with residents, families, physicians, and interdisciplinary team members.
• Provide compassionate, resident-centered nursing care while promoting dignity, independence, and quality of life.
• Supervise and collaborate with Certified Nursing Assistants to ensure exceptional resident care.
• Respond appropriately to emergencies and changes in resident condition.
