You will administer medications, perform health assessments, document care, and coordinate with physicians, families, and the care team to ensure safe, person-centered outcomes.

Qualifications

• LPN license current on Illinois HealthCare Worker Registry.
• Current BLS CPR certification for Healthcare Workers.
• Able to perform essential functions and meet environmental requirements.
• Ideal candidate has strong care ethic and works well in a fast-paced team.

Responsibilities

• Administer medications, tube feedings and treatments as prescribed.
• Prepare quarterly and annual health assessments.
• Inform physicians of changes and implement orders to ensure health care.
• Complete daily charting for provided care.
• Liaise between residents, families, physicians and case managers.
