Job summary

The Gardens of Arkansas is seeking a Licensed Practical Nurse (LPN) to support health, safety, and well-being of individuals receiving services. The LPN oversees medications, health monitoring, nursing documentation, care coordination, staff education, and communication with healthcare providers.

This role collaborates with the Nurse Liaison, Program Administrator, Clinical Director, APRN, physicians, pharmacy, DSPs, clients, and families to ensure compassionate, person-centered services.

Qualifications

• Current Arkansas LPN license.
• Graduate of an accredited practical nursing program.
• Current CPR and First Aid certification or ability to obtain certification within required timeframe.
• Ability to complete background, registry, drug screening, and exclusion screenings.
• Basic computer skills and ability to learn Therap, Microsoft 365, electronic MAR systems.
• Ability to read, understand, and implement healthcare provider orders.
• Strong documentation, communication, time-management, and organizational skills.
• Maintain confidentiality and exercise sound professional judgment.
• Able to work with individuals with behavioral health needs, intellectual or developmental disabilities, or co-occurring conditions.
• Valid driver’s license and reliable transportation.

Responsibilities

• Maintain medication rooms in clean, secure, and survey-ready condition.
• Review MARs for accuracy, omissions, and documentation errors.
• Ensure medications are properly labeled, stored, and available as ordered.
• Coordinate medication refills and communicate with pharmacies and providers.
• Assist with medication reconciliation after hospital visits or care transitions.
• Document nursing information accurately and timely in the electronic records.
• Provide staff education regarding medications and health needs.
• Follow nursing standards and assist with quality assurance and compliance.
