Job summary

Encompass Health is seeking an LPN/LVN to join our inpatient rehabilitation team. You will provide direct patient care, monitor vitals, and work closely with RNs to support individualized recovery plans.

Qualifications

• Current LPN/LVN licensure as required by state regulations.
• CPR certification.
• One year of inpatient medical-surgical or general hospital experience is preferred.
• Rehabilitation experience is preferred.

Responsibilities

• Provide direct patient care in accordance with the individualized care plan and physician orders.
• Observe and document patient vital signs, symptoms and responses to treatments.
• Collaborate with Registered Nurses to report findings and implement care plans.
• Build meaningful relationships with patients to support their rehabilitation and recovery goals.
• Celebrate patient victories and progress along the recovery journey.
