Encompass Health is seeking an LPN/LVN to join our rehabilitation team, providing compassionate, patient‑centered care close to home in Maine. You will work with Registered Nurses to implement individualized care plans and monitor patients during rehabilitative journeys.

Qualifications

• Current LPN/LVN licensure required by state regulations.
• CPR certification.
• One year experience in inpatient medical surgical or general hospital setting.
• Rehabilitation experience preferred.

Responsibilities

• Provide direct patient care according to individualized care plans and physician orders.
• Observe and document patient vital signs, symptoms and treatment responses.
• Collaborate with Registered Nurses to report findings and carry out care plans.
• Build therapeutic relationships with patients to support recovery.
• Support patient education and discharge planning where appropriate.
