Aviata Health Group is seeking a Licensed Practical Nurse (LPN) to join Aviata at Shoal Creek in Crestview, FL. The position includes day and night shifts in a family/team oriented environment within a skilled nursing center.

Responsibilities

• Assist in implementing individualized treatment plans for each resident.
• Supervise CNAs and participate in their evaluation.
• Oversee Falls and Pressure Sore programs.
• Provide nursing services and document patient status.
• Conduct and document evaluations of residents' medical status on admission and throughout treatment.
