Job summary
Elite Home Health Group INC in Spring Hill, FL is hiring a Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under RN direction. The role covers Spring Hill and surrounding areas and requires state licensure, CPR, and a valid driver's license.

Qualifications
- Graduate of an accredited Practical Nursing program.
- Current Florida LPN license in good standing.
- Minimum 1 year of nursing experience preferred.
- Home health, hospice, or acute care experience preferred.
- Current CPR certification.
Responsibilities
- Provide skilled nursing care to patients in the home setting.
- Administer medications and treatments within LPN scope of practice.
- Perform wound care, dressing changes, catheter care, and other skilled nursing tasks.
- Monitor patient condition and report changes to the supervising RN.
- Reinforce patient and caregiver education.
- Follow established plans of care and physician orders.
- Complete accurate and timely documentation in the EMR.
- Communicate with RNs, physicians, and other members of the care team.
- Participate in required training, meetings, and in-services.
- Provide care in accordance with Medicare requirements, state regulations, and agency policies.
