The position requires Alabama LPN licensure, a valid driver's license, and flexibility in hours. Preferred qualifications include CPR certification, home care experience, and strong communication skills.

Qualifications

• Current and unrestricted LPN licensure in Alabama.
• Valid driver's license and vehicle insurance, access to reliable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• Home care experience preferred.
• Ability to manage multiple tasks and work independently.
• Good communication, writing, and organizational skills.
• Flexible hours to meet client needs.

Responsibilities

• Provide high quality clinical services within the scope of practice as defined by Alabama license laws and in accordance with the plan of care.
• Document patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participate in care coordination, communicate with patient, family, physician, and care team.
• Function in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
