Job summary

Team Select Home Care of Colorado, LLC in Loveland, CO is seeking a compassionate Licensed Practical Nurse (LPN) for pediatric and/or adult in-home care. You’ll provide one-on-one nursing with an emphasis on safety, comfort, and family education, delivering skilled care under an RN, and documenting visits in our EMR system.

New graduates welcome; we offer structured training, flexible hours (days/nights, weekdays/weekends), weekly pay, and a supportive team environment with clinical supervision.

Qualifications

• Active LPN license
• CPR certification
• Excellent communication and critical thinking skills
• Willing to work in in-home care with pediatric/adult patients

Responsibilities

• Provide skilled nursing care under the direction of an RN
• Administer medications and treatments based on individualized plans
• Educate and support families in-home care techniques
• Observe and report changes in patient condition
• Document visits using EMR system
• Be part of a long-term care team focused on safety, comfort, and continuity
• Takes on-call duty, nights, weekends and holidays as assigned
