Job summary

Masonicare Home Health & Hospice in Norwich, CT, is seeking an LPN for homecare in the Norwich area. The position is day shift, 40 hours per week, with supervision by the Nurse Case Manager/Clinical Manager in home care settings.

The LPN will provide skilled nursing care in patients’ homes, hospitals, or extended care facilities, coordinating with a registered nurse and other team members to deliver effective care while ensuring documentation is complete and timely.

Qualifications

• Graduate of a NLN accredited school of nursing.
• Current State of Connecticut Practical Nurse license.
• One year of acute care experience required; homecare experience preferred.
• Possess a valid driver's license and reliable transportation.

Responsibilities

• Coordinate care with community and agency resources and with the designated RN on a regular basis.
• Manage adult, geriatric, pediatric, and neonatal client assignments; perform procedures and document appropriately.
• Communicate client care issues to the designated RN or CM in a timely manner.
• Support socialization needs and educate clients, families, and the community.
• Document timely and accurate patient/job information in the system for time sheets and reports.
• Communicate effectively with clients of various ages and cultures.
• Report to work on time and maintain required education and medical requirements.
• Perform other duties as assigned by the Clinical Manager/designee.
