Job summary

Wellstar Health System is seeking an LPN for home health in Georgia. The LPN provides direct patient care under RN supervision, coordinating care with the team and educating patients and families in their homes.

The role supports total patient needs across medical/surgical cases, participates in staff meetings, and may serve as a preceptor for new staff. Weekend visits and call rotation are part of the schedule.

Qualifications

• High School Diploma or GED General or Other / Certificate Nursing
• BLS certification required upon hire
• LPN licensure required

Responsibilities

• Assess and assist with managing patients total needs following physician orders with RN oversight
• Performs routine nursing procedures to meet the need of the patient under the supervision of RN
• Meets established productivity standards
• Coordinates care and services with RN and other team members
• Informs RN, therapist, and/or clinical supervisor of changes in patients condition
• Collaborates with RN to develop and revise individual plan of care based on patient need and physician order
• Perform other duties as assigned by Home Health leadership
• Serves as patient educator to foster self care or caregiver support
