Job summary

Prismhr Hire is seeking a licensed Practical Nurse to deliver patient-centered care in a collaborative health team. You will follow care plans, monitor conditions, and assist with assessments and education to promote safety and comfort.

The ideal candidate holds an approved LPN program, an active state license, and strong documentation and communication skills to support positive patient outcomes in a fast-paced setting.

Qualifications

• Current LPN license in the applicable state
• Graduate of an approved practical nursing program
• Experience in a clinical, long-term care, hospital, or outpatient setting preferred
• Knowledge of nursing principles, patient care practices, and safety standards
• Ability to document care clearly and communicate with the team
• Excellent interpersonal skills with a patient-focused approach
• Ability to prioritize tasks in a fast-paced environment
• Basic computer skills and familiarity with EHR

Responsibilities

• Provide direct nursing care in accordance with established care plans, policies, and procedures.
• Monitor patients’ conditions, record observations, and report changes to the clinical team.
• Administer medications and treatments as authorized and within scope of practice.
• Assist with routine procedures, patient assessments, and diagnostic preparations.
• Support patients with daily care needs while promoting comfort, dignity, and safety.
• Maintain accurate and timely documentation of patient care, interventions, and outcomes.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Follow infection control, safety, and regulatory standards at all times.
• Assist in patient education and reinforce care instructions as directed.
• Contribute to a positive and collaborative care environment.
