Qualifications

Responsibilities

• Providing direct patient care to residents, including administration of medications, wound care, and routine vital sign checks
• Assisting with personal care needs, such as bathing, dressing, and grooming
• Monitoring and reporting changes in patient condition to the healthcare team
• Participating in interdisciplinary care conferences to develop and implement individualized care plans
• Maintaining accurate and up-to-date documentation of patient care and treatment
