Qualifications

• Experience in Long Term Care settings.
• Proficiency with Point Click Care charting.
• Licensed LVN/LPN in the practicing state.
• BLS/CPR certification or willingness to obtain.
• Experience in nursing management is preferred but not required.

Responsibilities

• Follow established procedure for charting and incident reporting.
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient''s condition (nutrition, infections, skin, weight, etc.).
• Utilize the Quality Life Manual for identification, placement, and monitoring of patients.
• Document and notify applicable departments of patient room changes, LOA, new admits, or discharges.
• Make rounds with physicians when necessary.
