Job summary

Team Select Home Care is seeking a compassionate Licensed Practical Nurse (LPN) for pediatric and adult private duty nursing in home settings. Work includes one-on-one patient care, medication administration, and family education with a focus on safety and continuity of care.

New graduates are welcome; we offer structured training and mentorship. Flexible scheduling includes days/nights and weekdays/weekends with weekly pay and ongoing clinical supervision to support your growth.

Qualifications

• Average computer skills with competency in media communications.
• Must have the physical abilities to perform the described duties.
• Acceptance of vision, philosophy and goals of the Company.
• Ability to exercise initiative and independent judgment.
• Excellent verbal and written communication.

Responsibilities

• Furnishes services, treatments and diagnostic and preventive procedures as assigned per the plan of care, in accordance with agency policies.
• Prepares clinical and progress notes.
• Assists the physician, the intermediate care provider and RN in performing specialized procedures.
• Prepares equipment and materials for treatments observing aseptic technique as required.
• Assists the patient in learning appropriate self-care techniques.
• Understands and adheres to established policies and procedures.
• Follows/implements the nursing care plan for each patient as approved by the RN/physician.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Observes signs and symptoms and reports to the physician and RN nurse reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
• Teaches and counsels the patient and family regarding nursing care needs and other related problems of the patient at home.
• Maintains accurate and complete records of observations, treatments and care of patient.
• Participates in patient care conferences to discuss the need for involvement of other health team members.
• Provides effective communication to patient/family, team members, and other health care professionals.
