Excelsior Care Group in Deltona, FL is seeking a compassionate Licensed Practical Nurse (LPN) to join West Volusia Healthcare & Rehabilitation. This full-time position covers all shifts, with a pay rate up to $34/hour, and offers a team‑oriented environment focused on resident well‑being.

Responsibilities include administering medications, assisting with procedures, monitoring vital signs, wound care, recording care per regulations, and supporting CNAs to maintain high‑quality resident care.

Qualifications

• Graduate of an accredited school of nursing.
• Florida LPN license.
• CPR certification required.
• Minimum 2 years of experience in long‑term care or related settings.
• Strong clinical assessment, critical thinking, and teamwork skills.

Responsibilities

• Administer and accurately record medications and treatments.
• Assist physicians and healthcare professionals with diagnostic procedures and treatments.
• Observe and report residents’ vital signs, symptoms, and general condition.
• Provide direct nursing care, including wound care, IV therapy, and other clinical procedures.
• Assist with admissions, discharges, and transfers of residents.
• Document care in compliance with state and federal regulations.
• Communicate effectively with residents, families, and interdisciplinary team members.
• Ensure compliance with infection control and facility policies.
• Respond promptly to emergencies and implement appropriate interventions.
• Support CNAs and nursing staff to maintain high‑quality patient care.
• Participate in care plan meetings and contribute to resident assessments.
• Educate residents and families on health conditions, medications, and self‑care techniques.
