The role focuses on delivering high-quality, patient-centered care while coordinating with physicians, RNs, and other professionals to support positive outcomes. The ideal candidate will have an active LPN license in the state, completion of an approved PN program, and the ability to document care accurately in an electronic health record system.

Qualifications

• Current licensure as a Practical Nurse in the applicable state.
• Completion of an approved practical nursing program.
• Experience in a clinical, long-term care, hospital, outpatient, or similar healthcare setting preferred.
• Strong knowledge of nursing principles, patient care practices, and safety standards.
• Ability to accurately document care and communicate clearly with team members.
• Excellent interpersonal skills with a patient-focused approach.
• Ability to prioritize tasks and respond effectively in a fast-paced environment.
• Basic computer skills and familiarity with electronic health records preferred.

Responsibilities

• Provide direct nursing care in accordance with established care plans, policies, and procedures.
• Monitor patients' conditions, record observations, and report changes to the appropriate clinical team members.
• Administer medications and treatments as authorized and within scope of practice.
• Assist with routine procedures, patient assessments, and diagnostic preparations.
• Support patients with daily care needs while promoting comfort, dignity, and safety.
• Maintain accurate and timely documentation of patient care, interventions, and outcomes.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Follow infection control, safety, and regulatory standards at all times.
• Assist in patient education and reinforce care instructions as directed.
• Contribute to a positive and collaborative care environment.
