Job summary

Mercy is seeking a Licensed Practical Nurse (LPN) to provide direct patient care under the supervision of an RN. You will assist with admissions, assessments, and care coordination, while educating patients and documenting in the EMR.

You will collaborate with the care team to ensure safe, high-quality care for all patients. Requirements include graduation from an accredited LPN program and an active LPN license in your practicing state, with eligibility to meet State Board of Nursing standards.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current LPN license in the state of practice.
• Ability to meet all State Board of Nursing requirements.

Responsibilities

• Assist with patient admissions, discharges, and ongoing care.
• Perform patient assessments and monitor changes in condition.
• Communicate patient updates and concerns to the RN and care team.
• Participate in bedside handoffs and patient safety initiatives.
• Provide patient and family education and support.
• Document care accurately in the electronic medical record.
• Collaborate with the interdisciplinary team to deliver high-quality care.
• Delegate and direct support staff activities as appropriate.
• Participate in unit meetings, huddles, and process improvement activities.
• Perform additional duties as assigned.
