WF TPS is seeking a Licensed Practical Nurse (LPN) to deliver high-quality patient care for our long-term and short-term residents in Tennessee. The role includes administering medications, wound care, and monitoring vital signs as part of an integrated care team.

Responsibilities include developing individualized care plans, documenting progress, and participating in interdisciplinary conferences to ensure smooth transitions and optimal outcomes.

Qualifications

• Current LPN licensure.
• 0-2 years of experience in a post-acute care setting (new graduates welcome to apply).
• Strong communication and interpersonal skills.
• Ability to work collaboratively as part of a team.
• Basic computer skills and familiarity with electronic medical records.

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Document resident progress, report any changes or concerns to the healthcare team, and participate in interdisciplinary care conferences.
• Collaborate with other healthcare professionals to ensure seamless transitions and optimal outcomes.
• Maintain accurate and up-to-date medical records.
