Job summary

Amsterdam Nursing Home seeks a compassionate Licensed Practical Nurse to provide high-quality day-shift care to long-term and rehab residents in Manhattan.

The LPN administers medications and treatments, monitors vital signs and resident conditions, documents care accurately, and supports physicians and RNs with care plans; assists with ADLs and memory care, and responds promptly to changes in status within a resident-centered, culturally sensitive team.

Qualifications

• Must hold an active LPN license in NY.
• Experience in long-term or rehab care preferred.
• Ability to document care accurately.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor vital signs and resident conditions.
• Document care accurately in progress notes.
• Support physicians and RNs with care plans.
• Assist with activities of daily living (ADLs) and memory care.
• Respond promptly to changes in residents' status.
