Responsibilities

• Perform reassessments related to delegated interventions (not initial shift assessments).
• Notify supervising RN of changes in patient condition during delegated interventions.
• Provide reinforcement of patient and family education, including discharge instructions.
• Administer medications as approved per policy and as delegated by the supervising RN.
• Document data collected, medications administered and care provided.
• Collaborate with the RN to implement and modify the patient care plan.
