Home Health & Hospice Care in Merrimack, NH is seeking a Licensed Nursing Assistant for the Community Hospice House 2nd shift to deliver compassionate personal care under an RN.

You will provide personal care, assist with daily activities, monitor patient status, and document care while traveling within the service area.

Required: NH Nursing Assistant license in good standing, CPR certification, reliable and compassionate; preferred home care experience.

Qualifications

  • Licensed Nursing Assistant in good standing with NH Board of Nursing.
  • CPR certified and able to pass background checks.
  • Reliable, compassionate, and committed to patient-centered care.
  • Willing to travel within the service area to provide care.
  • Preferred: home care experience; dementia/COPD/cancer/post-surgical care; PT/INR checks.

Responsibilities

  • Provide personal care including bathing, dressing, grooming, toileting, and safe transfers.
  • Assist with light housekeeping, laundry, and meal preparation after care tasks.
  • Use mechanical devices and proper body mechanics to assist mobility safely.
  • Observe and report changes in patient condition and document promptly.
  • Cue patients for medications as directed by the care plan.
  • Maintain a clean, safe environment following aseptic and safety standards.
  • Communicate professionally with patients, families, and the care team.
  • Travel within the agency service area to provide care.
